Healer-Client Disclosure Form

In Compliance with California State Law

All Pranic Healers that reside in the State of California must
have their clients fill out a Disclosure Form prior to their session.

The Healer and the client must each have a copy of this form for
their records.

This Disclosure Form is to be used in conjunction with the
current Pranic Healing Session Record.
